About the Role

We’re expanding our Support Coordination team, and we’ve already got a caseload waiting for you—all you need to do is dive in and start helping those who need it most. Key Responsibilities
- Help participants understand and implement their NDIS plans.
- Connect individuals with services and community supports tailored to their needs.
- Monitor progress, troubleshoot challenges, and adjust strategies as required.
- Advocate for participants and ensure their voice is central to decision-making.
- Maintain accurate records and ensure compliance with NDIS guidelines.
About You
- Certificate IV or higher in Community Services (or related field).
- Strong understanding of the NDIS and person-centred practice.
- Excellent communication and problem-solving skills.
- Experience in disability or mental health services is highly regarded.
- Ability to manage competing priorities and work independently.
Mandatory Requirements
- National Police Clearance and 100-point identification check.
- Valid C Class WA Driver’s Licence.
- NDIS workers clearance and NDIS workers orientation module.
