Hybrid 1 Day Office & 4 days Across Two Adjoining SIL Houses

We are searching for a passionate Service Coordinator that is ready to lead, mentor and support a team that delivers caring, high-quality service delivery to our participants in our Supported Independent Living houses, otherwise known as Community Living here at Crosslinks.

About the Role:

Reporting to the Service Operations Manager, the Service Coordinator is responsible for overseeing service delivery across two community living houses, supporting a team of up to 14 Support Workers. This role ensures operations are efficient, compliant, and above all, person-centred. The Service Operations Manager will commit to continuous coaching, unwavering support and exposure that could lead to further career development opportunities.

Key Responsibilities:

  • Participant Support & Service Delivery: Ensure compliance with NDIS Practice Standards and safeguarding requirements are meet, ensuring participant goals are being developed and worked towards being achieved.
  • Team Leadership: Supervise a team of Support Workers, manage, build and develop rosters according to budgets and participants needs, coach staff on Doing What's Right, Not What's Easy, maintain performance standards through formal meeting and informal supports.
  • Compliance & Reporting: Maintain accurate participants documentation up-to-date, incident reports, and ensure adherence to policies. Ensure staff are compliance with WHS, Code of Conduct, NDIS Safeguarding requirements and mandatory training. 
  • Financial Awareness: Monitor house budgets and learn financial governance processes. 
  • Communication: Build strong relationships with participants, families, and staff to promote choice and control; conduct regular check-ins to demonstrate We Really Listen
  • Continuous Improvement: Identify service enhancements and support organisational initiatives.

About You:

You lead with empathy, listen, empower, and build trust, balancing strategic oversight with the everyday needs of your team. Most importantly, you believe disability support should be meaningful, uplifting, and bring joy to the lives of the people you support.

You Will Bring:

  • Relevant Certificate IV in Disability Support or Diploma of Community Services with minimum of 2 years in the disability sector.
  • Experience in NDIS Service Coordination desirable but not required.
  • Demonstrated leadership experience in disability, aged care, or community services.
  • Strong working knowledge of the NDIS Quality and Safeguarding Framework.
  • A values-driven approach to service delivery and team development.
  • Excellent communication, negotiation, and relationship-building skills.
  • Proficiency in Microsoft Office and confidence using specialist software.
  • Strong attention to detail and ability to work independently.
  • Current WA Driver's License and ability to travel.
  • Current NDIS Screening Check and orientation, Police Clearance, Working with Children Check, First Aid certification or willingness to obtain.
